Personal Health Risk Assessment forms now available in Profile

Family physicians can conduct a Personal Health Risk Assessment visit with their patients who belong to one of the at-risk populations (smoker, unhealthy eating, physically inactive, obese) either as part of proactive care or in response to a request for preventive care from a patient.

During this visit, the physician will complete a medical assessment and work with the patient to develop a health promotion and illness prevention plan that best suits the patient's medical needs and personal situation.

At the end of the visit, patients may identify a personal health or lifestyle change goal, which will be documented by the physician on a Prescription (Rx) for Health.
