BC CDM Toolkit: to be decommissioned June 2012
As you will recall from an earlier letter, the BC CDM Toolkit is being decommissioned on June 29, 2012. The Ministry of Health will automatically de‐activate all CDM Toolkit user accounts and the CDM Toolkit and associated patient data will no longer be accessible to you or your staff. Full technical support will continue as usual until that date.
Prior to June 29, 2012, please consider the following:
1. If you are using an EMR or plan to acquire one before June 29, 2012, contact the Physician Information Technology Office (PITO) as soon as possible.
PITO can assist you with conversion of your CDM Toolkit data so that your patient registries are established in the EMR and you can learn how the EMR can be integral to effective chronic disease management. In order to facilitate a smooth and timely transition from the CDM Toolkit to an EMR it is important to contact PITO as soon as possible. You can either contact the main PITO office for assistance; info@pito.bc.ca or 604‐638‐2946 or contact your local PITO Relationship Manager.
2. If you have patient data in the CDM Toolkit that you wish to retain and will not be converting to an EMR you will need to extract it. To do so there are two methods available; they are described below.
